If you are unable to complete a CIS within the 28 day permitted period, you can apply for an extension of time by completing and lodging this form with the WA Police Force.
Information about the CIS and how to make a booking can be found on the reverse of your CIR. If you do not complete a CIS, further police action will be taken against you.
