Applicant Authorisation

Your application must be supported by the following documents:

- A copy of your photo identification (e.g. drivers licence, passport, photo card).
- If you are applying on behalf of your organisation, a letter of consent on your company letterhead authorising you to obtain the information.

- A signed letter of consent from the victim authorising you to obtain information on their behalf. N.B. Representatives acting on behalf of an insurer, who in turn is representing the victim, must provide signed consent from the victim authorising you to obtain the information.

- A copy of your photo identification (e.g. drivers licence, passport, photo card).
- Documentary proof of representation (e.g. relevant representation order or certificate).
