Submit a Summons, Subpoena or Notice to Produce
This form can only be used for Summonses, Subpoenas and Notices to Produce issued to the Commissioner of Western Australia Police Force.
Note: this form may be rejected if:
- There is not sufficient time between service and the production date.
- The summons/subpoena is not endorsed by the court.
- A conduct fee is not provided.
To assist us in expediting your request, please provide as much detail as possible, including (if applicable):
- Full name and DOB (please describe the person’s relevance in your case).
- Incident Report number/s.
- Date and nature of the offence/s.
